Our staff has been trained under a medication therapy management model that involves a pharmacist providing four services, which are:

• Collecting medication-related information from the patient
   
• Assessing the collected information
   
• Reporting the assessment and findings to the primary care provider
   
• Referring to other members of the patient’s healthcare team, if necessary
